Facilities (CORF’s). A CORF provides comprehensive, community based accessible and cost effective rehabilitative care.
Our multi-disciplinary care teams include:
Patients must be referred by his or her physician. Please call us if you need help. We can coordinate care with your physician. Each patient will receive an individual plan of care with specific goals. Care is patient focused and one-to-one with a primary therapist. Care of every patient is regularly reviewed by our medical directors and care team. Our physician medical directors have experience and training in: orthopedic surgery, sports medicine, neurological disorders, and lung disease. |
